- Strengthening Personalized Medicine and Precision Public Health in the Context of Cardiometabolic Diseases (CMDs) in Colombia: A Translational and Convergent Approach.

This is a translational, biomedical research project,1,17,2 planned to contribute to move promising basic sciences knowledge towards a specific utility in CMDs, either individually (personalized medicine) or at a community level (precision public health).18,19 None of the experimental approaches described here involve clinical trials, but will certainly use human samples. Most of the projects within every WP belong to the category of pilot, cross-sectional studies, except the characterization of the UKBB Latino and Colombian which is a retrospective study.The project is focused in CMDs, the highest morbimortality and healthcare cost burden in the whole world.7,8,11,12 Unfortunately, some of the scientific advances made in developed countries need specific approaches to solve questions tan can only be solved under the specific conditions of the countries where they are intended to be applied. Such is the case of human genomics and its use as a prognosis and/or predictive tool, appliable to both individual and community levels, with the main goal of doing preventive medicine in chronic complex diseases.20,17,21,22 While most of the knowledge has been gained in developed countries with predominantly European ancestry population cohorts,21,22 very few have been carried out in more diverse and admixed populations as the ones in Colombia. Colombian populations are highly admixed as a result of the Columbian Exchange.23,24,25,26 A similar case occurs with the some viral-based therapies considered as promising tools in the context of CMDs.27,28 Given they are based of viruses that circulate in the population like AAVs and bacteriophages, the background immunity induced naturally might hamper their therapeutical effectiveness.29–32 Such background immunity has proven to be have shown strong regional and population variations and, therefore, they need to be assessed as part of their characterization as therapy tools in every population where they are considered for use.33,34 The project has been organized in two WPs. Methodologically, WP1 includes two genomics studies to be performed mostly in an in-silico research environment. It involves DNA isolation, genotyping, and bioinformatics. Given the scope of genomics, WP1 is focused on a wider spectrum of CMDs and is planned to be executed during the three years of funding. WP2 includes two seroepidemiological studies. This WP is to be performed in an in vitro scenario and planned to be executed since the second year of the project. Finally, both WPs involve local, national, and international collaborations, as well as opportunities for graduate and undergraduate students from the DCB and other academic areas at Uniandes (Figure S3). It is expected that this proposal allows a minimum of 5 scientific publications, including at least three manuscripts from WP1 and two from WP2.
